As far as I am aware, there aren't any lines that do both the Eastern and Western Mediterranean as a single cruise.
There is a 12 night cruise that starts from Venice (with an overnight in Venice on the ship)and finishes in Barcelona, stopping at the places you want to see except Cairo.

The vacations to go ticker will allow you to choose one port you wish to do and it will give you a list of cruises that hits that port. But they only allow you to choose one.

There aren't any cruises that will allow you to customize your cruise unless you do one of the smaller private cruises. Your best bet is to organize the ports you wish to do in an order of priority and call a travel agent that can spend the time looking for the ones that best fit your itinerary. Or check out the Vacations to Go site and spend the time digging around there.
